Custom Forms

Share a custom form with a direct link, embed it on any website, or add it to a page of your CauseVox site.

Understanding Custom Forms

A custom form collects answers to your own questions, such as a volunteer sign-up, an interest list, or an auction item donation form. It takes no payment.

Each submission saves the person's name, e-mail and answers, and creates or updates their CRM contact by e-mail. Custom forms are listed under Campaigns > Forms on the Custom tab.

Sharing a Custom Form

Open the form and go to Embed Code. There are three ways to share it.

The Direct Link is a page hosted by CauseVox that shows only this form. Copy it with Copy to Clipboard and share it by e-mail, social media, or a QR code. No code needs to be installed.

Embed on another website

Install the global JavaScript code on your website once, then paste the Embed the Form snippet where the form should appear. The Embed a Button snippet adds a link that opens the form in a pop-up instead.

Add to a CauseVox site page

In your site's page editor, browse modules, filter by Forms, and add the Custom Form module. In its settings, pick the form under Custom Form. See Site Modules.

Submissions and Export

Open the form and go to Submissions to see every submission, filter by Completed or Failed, and view one with the eye icon. Use the search box to find a submission by name or e-mail; the All, Completed and Failed tabs are hidden while a search is active.

Click the Export button (download icon) to download the form's submissions as an Excel or CSV file. Each question on the form gets its own column, in the same order as the form. Name, e-mail, status and the submission date are included too.

Address questions

An address question exports as one column holding the whole address, as stored. To get each part of the address in its own column, add the Line 1, Line 2, City, State, Zipcode and Country columns for that question from Available Columns. Your column choices are remembered for each form.

Completed exports appear under Data > Exports. See Quick Exporting.
